Refund Request Form

If you need to cancel your daughter's enrollment in the current program, a Refund Request Form needs to be submitted.  For the spring season this form must be submitted prior February 10th.

For the fall season this form must be submitted prior to August 10th.

Please understand that ALL cancellations maybe subject to a $25 processing fee.  Refunds granted may be done in the form of a credit to your Team Sideline account.
